Community Lead Nurse
My client has grown to become a leading provider of community based social care and support for thousands of vulnerable adults and children in England and Wales, focused on supporting people to live independently in their own homes and part of their local communities.
About the role
As a Community Lead Nurse, you will be undertaking care assessments and clinical risk assessments whilst providing advice and guidance to the wider business. You will be the clinical lead for complex packages in the areas covered, ensuring a team of Support Workers are both theory and practically trained and up to date in conjunction with specific client care. The role also requires shared on call responsibilities.
You will be working as part of a wide MDT including commissioners, GP's, family members and community professionals. The successful candidate will ensure that all issues relating to the safeguarding of vulnerable adults are reported according to policies and procedures.
There will be the opportunity to develop your own teaching skills and participate in staff education programmes and developing training profiles for each Complex Care client. The role will include Maintaining, reviewing and auditing all client clinical documentation in the service and at the CQC/CSSIW Registered Office.
